If you have potential conflicts to disclose, please submit the Disclosure Form for Civil Service Employees. Completed forms should be submitted to your supervisor for approval, and then sent to the Office of the Vice President for Academic Affairs (OVPAA). Note that the civil service form should be used in place of the form that academic professionals submit, the Report of Non-University Activities or RNUA (UIUC/UIS) or the Disclosure Profile (UIC).
